Instructions:

1. Mark how many you want of each item in the appropriate box and then total your order.
2. Attach cash or check (payable to CSS PCO) and give it to your child’s teacher or put in PCO locked mailbox in the office (mounted on the wall just to your left as you walk in the door).
3. Your items will be delivered in your student’s backpack.

SHIPPING:

If you do not have a student at Commodore Sloat and want to order Sloat Wear, you can mail in your order and either give us your address and donate a little extra to offset the cost of shipping, or make arrangements by e-mail for picking your item up in person (e-mail sloatparents@gmail.com).
